Purpose: To identify predictors of a switch to mania in depressed patients treated with paroxetine. Study design: Retrospective chart review. Follow up: Variable. Patients: 79 depressed outpatients (unipolar, pseudo-unipolar, or bipolar depression). Treatment: Paroxetine 10–40 mg/day. Results: Switching to mania occurred in 9% of patients treated with paroxetine. Bipolar depression (odds ratio = 20) and family history of psychiatric illness (odds ratio = 11) were associated with a greater risk of switching.
